Finding a highly qualified professional is an important first step in your healthcare journey. The Center provides you with your very own Care Team of board – certified physicians in Family, Internal Medicine and Pediatrics, Family Nurse Practitioners, Family Physician Assistants, Certified Medical Assistants, and other healthcare staff that ensure you receive the best care possible. All our healthcare staff work in a team environment, serving patients of all ages and medical backgrounds and ensuring we meet each patient ’ s individual needs. Your healthcare provider plays a crucial role as a diagnostician who listens and reacts to your healthcare needs.

Coordinated Care
We coordinate care across many settings. We are concerned about the “whole person” and take responsibility for coordinating services for you and your family.

Support & Coordination
Our care team will help you access, coordinate and understand specialty care, educational services, self care, family support, and other public and private community services.

Dedicated Clinician
You will select a specific clinician. Along with his/her care team, your clinician is responsible for providing continuity of care and coordinated care for you and your family.

Well & Sick Visits
Schedule well visits with your selected clinician and acute care/sick visits with your clinician when possible by calling 703-680-7950.

After-Hours Care
Providing clinical advice to patients during and after hours. Call 703-680-7950 for assistance. Patients with significant after-hours health care concerns should call 911 or go to the nearest Emergency Room.

Full Medical History
We ask patients to share complete medical history and details about care received outside of our Center.

Include Key Information
This includes medicines, medical history, test results, and hospitalizations or visits to the ER or specialists.

Your Responsibility
While we will ask for this information, it is your responsibility to provide complete and accurate details so we can deliver the best care for you and your family.
