STEPS TO SEE YOUR REFERRAL

• You will receive a text message from Lunera Health
• Click on the link in blue
• Enter your information (FIRST NAME, LAST NAME and DATE OF BIRTH)
  Please make sure you enter your DATE OF BIRTH with month first, then day second and then the year last. Ex: 05/20/1960
• Once you are in your chat, click on “File attached” to see your referral
• Save your referral to your phone. Your provider wants you to get help right away.
